Dr. Sola is delighted to connect with you in person or via telemedicine. She would like for you to fill out a New Patient Intake Form before you schedule for your appointment. It’s pertinent for her to prepare for your visit so that your time is used wisely and efficiently. Please have the intake form completed at least 48 hours (2 days) in advance to your booked appointment.

CANCELLATION POLICY

There is a 24-hour cancellation policy.  Late cancellations/no-shows will be billed at the following rates: $100 for a new patient visit and $50 for a follow-up visit. Missed appointments without any notification will be billed in full. We understand that emergencies occur and are willing to make exceptions for true emergencies. We do require a credit card in order to reserve your appointment.
